Health Information Management at Summersville Memorial Hospital is dedicated to the effective management of patient information and health care data needed to deliver quality treatment and care to the public. The move from paper to electronic health records will help doctors, nurses and other medical staff make important healthcare decisions on a real time basis. But who ensures the information is complete, accurate, and kept confidential? This is the role of the health information management professional.

Medical records may be inspected, or copies furnished when the following procedure is followed:

1 - Upon receipt of written authorization signed and dated by the patient, guardian, if a minor, conservator if mentally incompetent, administrator or executor, in case of death.

2 - In order to pick up copies of records identification must be shown by the party wishing to inspect and/or request copies.

If a patient would like to obtain copies of their records for another physician these will be given at no charge for the first time. Additional requests will have fees.
